Insurance Carriers and Forms

If you are a new patient or have not been here for a while, we ask that you provide us with some information. This can either be picked up ahead of time, faxed, or filled out at the date of initial service. This information will allow us to contact you if necessary and to assist us in filing insurance claims. We also ask that you bring along your insurance card when you come for your appointment.

As a courtesy, our business office will file your claims directly with your insurance company. However, many insurance plans require you to satisfy your deductible, copay, and/or a coinsurance as part of your coverage. Although clients should understand their insurance benefits and coverage, our office staff will verify insurance coverage for your treatment and discuss any financial obligation that exists. If any questions arise, our staff is available to provide assistance.

Dakota Physical Therapy is a participating In-Network provider for Blue Cross Blue Shield, DakotaCare, Sanford, Avera, Medica, United Healthcare, Humana, Medicare, Medicaid, and over 70 other insurance companies. If you have a question about whether your insurance is covered at Dakota PT, please call and our staff will be happy to verify coverage.

Avera Health Plan Members

After years of tireless efforts by the private practitioners of South Dakota, Dakota Physical Therapy is an In-Network provider for Avera Health Plans (with the exception of Avera Employees-refer to topic below). Your physical therapy benefit is the same at our clinic as it is at any Avera facility, and we will continue to provide the same highest quality care and services that we have always provided to you in the past.

 

Avera Employees

Avera Health Plan has informed their employees that as of January 1st, 2016, Dakota Physical Therapy will be considered out-of-network.  This applies to AVERA EMPLOYEES WITH AVERA INSURANCE AND THEIR FAMILY MEMBERS WITH AVERA INSURANCE, NOT to everyone who has Avera Health Plan coverage.

If you are an Avera employee or a family member of a Avera employee who has Avera health insurance and you would like to see a therapist at Dakota PT, you should follow up with your human resources and benefits office to express your concerns and ask about what options might be available to you to seek and continue your care at Dakota PT.

Please refer to the following link for Avera's written policy on out-of network providers. Keep in mind, even though the information indicates out-of network providers do not participate with Avera Health Plans, we have tried to apply for coverage and Dakota PT is not being allowed to participate.
